Benefit Verification Guidelines
Dealing with insurance companies regarding your infertility coverage might seem intimidating at first, but with the right tools and information, we can help guide you through the process. Insurance companies have specific guidelines to help you determine the extent of your fertility benefits and they are obligated to provide you this information.
Be sure to obtain written verification of your benefits.
To Verify Your Benefits:
- Call the customer service phone number printed on your insurance card.
- Ask specifically for any benefits that cover infertility
- Request written confirmation of these benefits
- Obtain the name of the representative assisting you
- Document the date and time of your call
- Review your benefits booklet which may be provided by your employer. If you have any infertility benefits, copy this section of the booklet to give to our billing office.
- If treatment benefits are available on your insurance plan, you will need to supply written confirmation of benefits to our business office. This may be provided either in letter format from your insurance carrier, or by copies from your benefits book.
